Liability Waiver & Release
In consideration of being allowed to use the facilities at BG Fun Center, I, the undersigned parent/legal guardian, acknowledge the risks inherent in active play and agree to the following: (1) I certify the listed children are in good health. (2) I assume all risk of injury arising from play. (3) I release BG Fun Center, its owners, and employees from all liability to the fullest extent permitted by law. (4) All children must wear non-slip socks at all times (available for purchase). (5) I agree to supervise my children while in the facility. This waiver remains in effect for all future visits until revoked in writing.
